About The Position
Concierge Home Care is hiring a Home Health Physical Therapist Assistant to provide one-on-one care in patients’ homes in Sebastian, Florida, and Indian River County. The role offers a pay-per-visit compensation model with additional earning potential, consistent patient volume, and territories designed to minimize drive time through clustered scheduling. The position provides autonomy to create and manage a daily schedule, with full-time, PRN, and modified schedule options. PTAs receive clinical support through structured onboarding, mentorship, preceptorship, ongoing training, and career advancement opportunities. Documentation uses AI-assisted tools and the WellSky/Kinnser EMR. Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist Assistant (PTA) license.
- Valid driver’s license, reliable transportation, and auto insurance.
- Successful completion of an AHCA Level 2 background screening is required for employment.
Nice To Haves
- Home health experience is preferred but not required.
Responsibilities
- Provide hands-on therapeutic care according to the plan of care established by the Physical Therapist.
- Assist patients in improving mobility, strength, and independence.
- Collaborate with patients, caregivers, and interdisciplinary teams.
- Monitor patient progress and communicate updates to the supervising Physical Therapist.
- Complete documentation using streamlined, AI-assisted tools.
