About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to serve patients in Kingsport, Tennessee, covering Grainger and Claiborne Counties. The role provides expert, patient-centered care in patients’ homes and offers independent work with team support. The schedule is full-time, Monday through Friday, 8:00 a.m. to 5:00 p.m. Homecare Homebase (HCHB) experience is required. A $7,500 sign-on bonus is offered.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification that is maintained.
• Experience with Homecare Homebase (HCHB).

Nice To Haves

• Community or hospice experience is preferred.
• New physical therapy graduates may be considered in select markets based on program availability.

Responsibilities

• Evaluate and treat patients through physical therapy based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication with the treatment team, physician, patient, and family members as applicable.
• Complete thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities in accordance with the state practice act.
