About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Kingsport, Tennessee, covering Grainger and Claiborne Counties. The role provides one-to-one care in patients’ homes, with flexible scheduling options, independent work, and team support. The position is full-time, Monday through Friday, 8:00 a.m. to 5:00 p.m. A $7,500 sign-on bonus is offered. Experience with Homecare Homebase (HCHB) is required.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist; new graduates may be considered in select markets based on program availability.
• Experience with Homecare Homebase (HCHB).
• Valid license in the state of practice.
• Current CPR certification.

Nice To Haves

• Community or hospice experience is preferred.

Responsibilities

• Perform physical therapy evaluations and provide treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients achieve their maximum level of independence and function.
• Maintain communication with the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities in accordance with the state practice act.
