About The Position

Health Link Home Health & Hospice is seeking a Physical Therapist to serve patients across South Bay neighborhoods in Santa Clara County. The role offers autonomy to manage patients, create plans of care, and practice independently, with support from an experienced clinical team, case managers, and care coordination staff. Requirements

Current California Physical Therapist license.
Valid California driver's license.
Strong clinical judgment, organization, and time-management skills.

Responsibilities
