About The Position

This is a travel assignment for a Physical Therapist in an outpatient setting located in Lebanon, Oregon. The contract is for 13 weeks, starting on 09/28/2026. Requirements

Current unencumbered Oregon Physical Therapy license
Healthcare Provider Level BLS certification
Ability to travel to multiple points of care
Strong clinical documentation and communication skills
Ability to work in a hospital-based outpatient rehabilitation environment

Outpatient rehabilitation experience preferred

Responsibilities

Conduct initial evaluations and ongoing treatment for outpatient patients with injuries or conditions affecting mobility and functional independence.
Develop, implement, and adjust individualized rehabilitation plans; monitor progress and modify interventions as needed.
Provide therapy interventions such as therapeutic exercise, gait training, manual therapy, and modalities within scope and department guidelines.
Document patient assessments, treatment plans, goals, and progress in the medical record; communicate with the care team and document patient education provided.
Educate patients and families on home exercise programs, safety strategies, and functional adaptations to support continued recovery.
Collaborate with a multidisciplinary team to coordinate care and ensure seamless patient transitions across services.
