About The Position

Talin Home Health is seeking a dedicated and skilled Physical Therapist to evaluate and treat patients in their homes, helping improve physical function and quality of life. The therapist will collaborate with other healthcare professionals to develop care plans tailored to each patient’s needs. The position is focused on the Rappahannock areas; other areas may be available upon request. Work is on the road.

Requirements

A degree in Physical Therapy from an accredited institution.
Strong knowledge of medical terminology and anatomy.
Excellent interpersonal skills to build rapport with patients and collaborate with healthcare teams.

Nice To Haves

Clinical experience, particularly in orthopedics or home community care, is preferred.
Familiarity with discharge planning processes and systems used in patient care.

Responsibilities

Evaluate patients’ physical conditions and functional abilities.
Develop individualized care plans addressing each patient’s needs.
Implement treatment programs using therapeutic techniques such as exercises, manual therapy, and modalities.
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
Educate patients and families about rehabilitation, home exercises, and injury prevention.
Collaborate with interdisciplinary teams to support comprehensive care, including discharge planning for inpatient cases.
Document patient evaluations, treatments, and progress in accordance with clinic policies.
