About The Position
Boost Home Healthcare is hiring a Home Health Physical Therapist to provide personalized, coordinated physical therapy to patients in their homes in Fort Collins, Colorado, and surrounding areas. The role offers full-time, part-time, or PRN scheduling, with a day shift schedule Monday through Friday and weekends as needed. Pay is $68.00–$76.00 per visit. The Physical Therapist provides physician-prescribed care to help patients improve mobility and functional independence. Responsibilities include assessing patients, creating individualized care and maintenance plans, documenting care according to Medicare and OASIS protocols, collaborating with an interdisciplinary care team, and supervising Physical Therapist Assistants and support personnel as needed. Requirements
- Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited APTA/CAPTE program.
- Hold an active Physical Therapist state license in good standing.
- Demonstrate strong clinical assessment skills, a compassionate patient-first mindset, and excellent communication abilities.
- Be proficient with Medicare OASIS documentation and EHR software.
Nice To Haves
- At least 1 year of clinical physical therapy experience is preferred.
- Home Health experience is highly preferred.
Responsibilities
- Provide physician-prescribed physical therapy in patients’ residences to improve mobility and functional independence.
- Perform initial evaluations, establish personalized plans of care, and develop maintenance programs.
- Conduct 30-day reassessments.
- Participate in patient care planning with nurses, physicians, and home care staff.
- Complete accurate clinical documentation in accordance with Medicare guidelines and OASIS protocols.
- Direct and supervise Physical Therapist Assistants and support personnel as required.
