About The Position
Metro Physical & Aquatic Therapy is seeking a motivated, passionate per diem Physical Therapist to provide physical therapy in patients’ homes in Glen Cove and surrounding towns. The position welcomes physical therapists at all experience levels and offers training for new graduates. Home health experience is preferred but not required; a mentor will support documentation and compliance. The role offers a flexible schedule, with the option to see patients any day of the week, and preference is given to clinicians serving their primary geographic area. Compensation is $70–$90 per visit, with the posting also describing compensation as hourly. 1099 and W-2 work arrangements are available.
Requirements
- Graduate of a Physical Therapist program.
- Licensed as a Physical Therapist in New York or eligible for licensure.
- Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities
- Accept and treat patients within the designated geographic area.
- Evaluate patients’ physical therapy needs in the home setting and develop individualized treatment plans.
- Develop plans of care and treatment goals in consultation with physicians, patients, and families.
- Plan, prepare, and implement customized therapy programs to restore function and promote independence.
- Instruct family members, PTAs, home health aides, and caregivers on appropriate therapy techniques.
- Provide timely and accurate documentation for each visit.
- Communicate effectively with patients, families, and interdisciplinary team members, particularly regarding discharge planning.
- Deliver care in compliance with agency policies and professional practice standards.
