About The Position

TRU Healthcare is hiring a per diem Physical Therapist to provide one-on-one care to patients in their homes. The role focuses on helping patients improve mobility, strength, balance, functional independence, and quality of life through individualized treatment. The clinician manages visits in coordination with patient needs and their availability, and collaborates with patients, families, caregivers, nurses, occupational therapists, aides, and physicians. The position offers per-visit compensation, mileage reimbursement, scheduling flexibility, autonomy, and AI-powered documentation tools. Per diem positions may have different benefit eligibility than regular full-time positions.

Requirements

• Current Minnesota Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• A genuine passion for helping patients achieve their goals and maintain independence.
• Strong assessment, treatment planning, and clinical decision-making skills.
• Ability to work confidently and effectively in a home health environment.
• Willingness to collaborate with patients, families, caregivers, and the interdisciplinary team.
• Valid driver's license and reliable transportation for travel between patient visits.

Responsibilities

• Evaluate and treat patients in their homes and develop individualized treatment plans focused on mobility, function, independence, and quality of life.
• Manage patient visits and accept work in line with availability and patient needs.
• Help patients improve strength, balance, mobility, and functional independence.
• Collaborate with nurses, occupational therapists, aides, physicians, patients, and families to coordinate care.
• Educate patients and caregivers about therapeutic exercises, mobility, safety, and strategies to support progress at home.
• Complete timely, accurate clinical documentation and communicate patient progress and changes in condition to appropriate care team members.
