About The Position
TRU Healthcare is hiring a per diem Physical Therapist to provide one-on-one home health care to patients in their homes. The role focuses on improving patients’ mobility, function, strength, balance, independence, and quality of life through individualized evaluation and treatment. The Physical Therapist manages visits according to their availability and patient needs, collaborates with an interdisciplinary care team, educates patients and caregivers, and documents care and patient progress. The position offers $100 per routine visit, mileage reimbursement, scheduling flexibility, and AI-powered documentation tools. Benefits eligibility may differ from that of regular full-time positions.
Requirements
- Current Minnesota Physical Therapist license.
- Graduation from an accredited Physical Therapy program.
- Genuine passion for helping patients achieve their goals and maintain independence.
- Strong assessment, treatment-planning, and clinical decision-making skills.
- Ability to work confidently and effectively in a home health environment.
- Willingness to collaborate with patients, families, caregivers, and the interdisciplinary team.
- Valid driver's license and reliable transportation for travel between patient visits.

Responsibilities
- Evaluate patients in their homes and develop individualized treatment plans.

- Help patients improve strength, balance, mobility, and functional independence.
- Coordinate care with nurses, occupational therapists, aides, physicians, patients, and families.
- Educate patients and caregivers about therapeutic exercises, mobility, safety, and strategies for continued progress at home.
- Complete timely, accurate clinical documentation.
- Communicate patient progress and changes in condition to appropriate care team members.
