About The Position
Sage Group HealthCare Staffing is seeking a Physical Therapist (PT) for a 13-week travel/contract position in Everett, Washington. This role involves providing Physical Therapy services to Home Health Patients, consulting with physicians and interdisciplinary teams to develop treatment plans, and utilizing skills in patient evaluation and treatment to maximize patient independence. The PT will create and implement treatment plans based on patient data, provide in-home PT treatment programs to restore function, relieve pain, and prevent disability, and help patients and their families maintain dignity and independence in their homes. Responsibilities also include driving to patient homes within assigned territories (primarily Lake Stevens/Snohomish area, within a 50+ mile radius from the office in zipcode 98203) on a daily basis and completing charting and patient documentation in a timely manner according to client policies and procedures.
Requirements
- Physical Therapist: 1-2 years - Required
- Recent/Current Home Health OT experience: 1 year within the last 3 years - Required
- Home Care Home Base (HCHB) charting - Required
- Must have valid Driver’s License and Personal Auto Insurance
- PT License for State applying or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Responsibilities
- Provide Physical Therapy services for Home Health Patients
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on a daily basis.
- Complete charting and patient documentation in timely manner as per client policies and procedures
Benefits
- Pay Rate: $2800 total weekly gross - $70/hour($31/hour W - 2 taxable , $44/hour non taxable stipend)
