About The Position

Charter Oak Home Care is seeking multiple experienced caregivers, including PCAs, HHAs, and CNAs, to care for older adults in their homes in the Kent, Salisbury, and New Milford areas. Caregivers help implement personalized care plans and schedules, provide personal and companionship care, and support clients’ safety, independence, and quality of life. Shifts range from 4 to 12 hours and are available across all days of the week, with first- and second-shift hours. Caregivers may design a flexible part-time or full-time schedule, with openings ranging from 4 to 40 hours per week. Charter Oak Home Care is a family-run organization established in 1985. Requirements

Compassion and empathy.
Ability to use good judgment and remain calm.
Reliability and punctuality.
Proof of eligibility to work in the United States.
Ability to pass a background check and provide references.
Experience as a caregiver, such as a PCA, HHA, or CNA.

Alzheimer’s or dementia experience.

Responsibilities

Deliver exceptional care to clients in their homes.
Help execute comprehensive, personalized weekly schedules and care plans.
Provide companionship and ongoing social and emotional support, staying actively engaged with each client.
Assist with personal care, including hygiene, mobility, light housekeeping, feeding, and meal preparation.
Keep order, safety, and client enjoyment top of mind.
Provide transportation to and from appointments as needed.
Document completed tasks and report changes in the client’s condition.
