About The Position

Charter Oak Home Care is seeking multiple experienced caregivers, including PCAs, HHAs, and CNAs, to support older adult clients in their homes in the Kent, Salisbury, and New Milford area. Caregivers help implement personalized care plans and schedules, provide personal care and companionship, and support clients’ quality of life and independence. Shifts range from 4 to 12 hours on all days of the week, with first- and second-shift hours available. Caregivers may design a flexible part-time or full-time schedule.

Requirements

• Proof of eligibility to work in the United States.
• Ability to pass a background check and provide references.

Responsibilities

• Deliver exceptional care to clients in their homes.
• Help execute personalized weekly schedules and care plans.
• Provide companionship and ongoing social and emotional support.
• Assist with personal hygiene, mobility, light housekeeping, feeding, and meal preparation.
• Keep client order, safety, and enjoyment in mind.
• Provide transportation to and from appointments as needed.
• Document completed tasks and report changes in clients’ conditions.
