About The Position

Charter Oak Home Care is seeking experienced caregivers, including PCAs, HHAs, and CNAs, to provide care for older adults in their homes. Caregivers help implement personalized care plans and schedules, provide companionship and social and emotional support, assist with daily personal care, and help clients remain safe and engaged. The role offers flexible scheduling, with shifts ranging from 4 to 12 hours and opportunities to work part-time or full-time. Hours are available from 4 p.m. onward, Monday through Friday, with weekend shifts also available.

Requirements

• Compassion and empathy.

• Reliability and punctuality.
• Proof of eligibility to work in the United States.
• Ability to pass a background check and provide references.
• Caregiver experience as a PCA, HHA, or CNA is sought.

Responsibilities

• Deliver exceptional care to clients in their homes.
• Help implement comprehensive, personalized weekly schedules and care plans.
• Provide companionship and ongoing social and emotional support.
• Assist with personal hygiene, mobility, light housekeeping, feeding, and meal preparation.
• Prioritize client safety, order, and enjoyment.
• Provide transportation to and from appointments as needed.
• Document completed tasks and report changes in clients’ conditions.
