About The Position

Charter Oak Home Care is seeking an experienced caregiver to support a male client with late-stage dementia in his home. The client lives with his wife, is bedbound, uses a Hoyer lift for transfers, is at risk of falls, and has a catheter. The caregiver will assist with personal care and implement care plans that support the client’s physical and emotional well-being. The listed schedule is Friday from 3 p.m. to 7 p.m. and Saturday from 11 a.m. to 7 p.m. Other openings may offer flexible schedules of 4–12-hour shifts across the week, with caregivers able to design a part-time or full-time schedule. Charter Oak Home Care is a family-run organization founded in 1985 that serves Connecticut seniors who wish to remain independent in their homes. The caregiver will provide personalized care, companionship, and support with scheduled activities and overall care management, while helping enhance clients’ quality of life.

Requirements

Compassion and empathy.
Ability to use good judgment and remain calm.
Reliability and punctuality.
Proof of eligibility to work in the United States.
Ability to pass a background check and provide references.
Experience as a PCA, HHA, or CNA is sought.

Responsibilities

Deliver care to the client in their home.
Help implement personalized weekly schedules and care plans.
Provide companionship and ongoing social and emotional support.
Assist with personal hygiene, mobility, light housekeeping, feeding, and meal preparation.
Keep safety, order, and client enjoyment in mind.
Provide transportation to and from appointments as needed.
Document completed tasks and report changes in the client’s condition.
