Partner Manager

Proactive Health•San Antonio, TX
•$70,000 - $85,000•Onsite

About The Position

Proactive Health is an early-stage HealthTech company focused on improving outcomes for seniors through biometric monitoring and human-centered care. The Partner Manager owns day-to-day relationships with senior care partners in an assigned territory, including skilled nursing facilities, home health agencies, and other organizations. The role focuses on making partner relationships productive and durable by enabling referrals, removing process barriers, maintaining referral cadence, and growing patient volume. For enterprise accounts spanning multiple territories, the Partner Manager coordinates with a senior relationship owner responsible for executive sponsorship and broader account strategy. The Partner Manager independently manages their calendar, schedules meetings with current and prospective partners, and builds and maintains a territory pipeline. The position also includes hands-on in-home patient device installations in new or developing territories until volume supports a dedicated installer, with cross-training to support installs across territories as needed. After building a full portfolio, the role's effort is expected to shift to approximately 70% partner expansion and enablement and 30% new partner development. Performance is measured primarily by patient referral volume and referral-to-activation conversion, with partner health, retention, onboarding velocity, and CRM hygiene as secondary indicators. The position is based in San Antonio, Texas, with regular travel within the territory estimated at 60–70% field-based. Compensation is location-adjusted. For San Antonio, the listed base salary is $70,000–$85,000, variable compensation at target is $75,000, and on-target earnings are $145,000–$160,000. Variable pay is uncapped and tied to referral volume and referral-to-activation conversion. Mileage reimbursement is provided at 76% of total business miles driven, processed monthly. Employment is contingent on a background check and motor vehicle record review after offer acceptance and before the start date.

Requirements

  • 3 to 6 years of experience in account management, partner management, or field sales within healthcare, senior care, home health, health technology, DME, or pharmaceutical sales.
  • An established network of active contacts in the territory's senior care community, such as home health, skilled nursing, assisted living, physician practices, or discharge planning.
  • Demonstrated success managing and growing B2B or B2B2C relationships with measurable outcomes.
  • Assertive, business-minded approach, with the ability to identify opportunities and move accounts forward independently.
  • Comfort working in clinical environments and building credibility with administrative and clinical stakeholders.
  • Comfort working directly with seniors and their families in the home and willingness to perform hands-on patient device installs when needed; training is provided.
  • Strong in-person presence and ability to build trust.

  • Excellent verbal communication, follow-through, and organizational skills.
  • Reliable personal vehicle with current registration, a valid driver's license, and auto insurance meeting state minimum requirements throughout employment.
