Partner Manager

Proactive Health•Denver, CO
•$150,000 - $165,000

About The Position

Proactive Health is hiring a Partner Manager to own day-to-day relationships with senior care partners in an assigned territory. The role focuses on growing referrals, improving referral-to-activation conversion, enabling partner staff, and maintaining productive, durable relationships. The Partner Manager independently manages their calendar and pipeline, schedules partner meetings, and works with a senior relationship owner on enterprise accounts that require executive sponsorship or span multiple territories. The role balances expansion and enablement within existing partner accounts with development of new partners. Once the portfolio is full, the expected allocation is approximately 70% of effort on existing partners and 30% on new partner development. In new or developing territories, the Partner Manager may perform in-home patient device installations until volume supports a dedicated installer and may provide installation support across territories as needed. Success is measured primarily by patient referral volume and referral-to-activation conversion, with partner health, retention, onboarding velocity, and CRM hygiene as secondary indicators.

Requirements

  • 3 to 6 years of experience in account management, partner management, or field sales in healthcare, senior care, home health, health technology, DME, or pharmaceutical sales.
  • An established network of active contacts in the territory's senior care community, such as home health, skilled nursing, assisted living, physician practices, or discharge planning.
  • Demonstrated success managing and growing B2B or B2B2C relationships with measurable outcomes.
  • Assertive, business-minded approach and ability to identify opportunities and drive accounts forward independently.
  • Comfort working in clinical environments and building credibility with administrative and clinical stakeholders.
  • Comfort working directly with seniors and their families in the home and willingness to perform hands-on patient device installs when needed; training is provided.
  • Strong presence in in-person settings and ability to build trust.
  • Fluency in Salesforce; proficiency is strongly preferred.
  • Excellent verbal communication, follow-through, and organizational skills.
  • Reliable personal vehicle with current registration, valid driver's license, and auto insurance meeting state minimum requirements throughout employment.
  • Willingness to travel regularly within the territory; estimated 60 to 70% field-based.

Nice To Haves

  • Background in post-acute care, home health, hospice, or senior living sales or operations.
  • Experience selling or supporting a SaaS or device platform in a healthcare setting.
  • Experience with in-home care, medical device setup, or field service support.
  • Familiarity with Medicare, discharge planning workflows, or care transitions.
  • Proficiency across Google Workspace, including Gmail, Docs, Sheets, Calendar, and Drive.

Responsibilities

  • Own day-to-day relationships with skilled nursing facilities, home health agencies, and other senior care partners in the territory.
  • Grow referral volume within existing accounts by identifying referral pathways, educating partner staff, and reducing referral barriers.
  • Conduct business reviews with partner administrators and clinical leads to review referral trends, resolve friction, and align on growth goals.
  • Serve as the primary contact for partner escalations and coordinate resolution with internal clinical and operations teams.
  • Build relationships with clinical staff, case managers, and administrative leaders across partner accounts.
  • Coordinate with senior relationship owners on enterprise accounts to align local enablement and referral activity with broader account strategy.
  • Schedule and lead expansion touchpoints using in-person visits, virtual meetings, and on-site educational events.
  • Track partner health, referral cadence, and conversion metrics in CRM and flag accounts at risk of churn.
  • Identify and prospect prospective partner organizations, schedule introductory meetings, and lead educational discussions about Proactive Health's services.
  • Qualify and onboard new partner accounts, including coordinating kickoff education sessions for partner and internal teams.
  • Maintain a pipeline of new opportunities in CRM and provide forecasts to the manager.
  • Represent Proactive Health at local healthcare industry events, trade associations, and continuing education forums.
  • Perform in-home patient device installs in new or developing territories as needed, including device setup, connectivity checks, and patient and family orientation.
  • Complete installation cross-training and remain current on installation procedures to support installations across territories as needed.
  • Coordinate installation scheduling with the Patient Install Scheduling team and maintain accurate installation status in Salesforce.
  • Provide a warm, patient-first experience in the home.
