Partner Manager

Proactive Health•Lehi, UT
•$70,000 - $85,000

About The Position

Proactive Health is an early-stage HealthTech company focused on improving outcomes for seniors through biometric monitoring and human-centered care. The Partner Manager owns day-to-day relationships with senior care partners in an assigned territory, including skilled nursing facilities, home health agencies, and other organizations. The role focuses on enabling partners, increasing referrals, resolving friction, and maintaining a productive and growing partner portfolio. For enterprise accounts spanning multiple territories, the Partner Manager coordinates with a senior relationship owner responsible for executive sponsorship and strategic expansion. The Partner Manager independently schedules partner and prospect meetings, manages territory activity and pipeline, and is measured primarily on patient referral volume and referral-to-activation conversion. In new or developing territories, the role also performs in-home device installs until volume supports a dedicated installer, and remains cross-trained to assist with installs as needed. Once a full portfolio is established, the role is expected to spend approximately 70% of its time on existing-partner expansion and enablement and 30% on new partner development. The position is based in the Salt Lake City South, Utah territory and is full-time. Compensation is market-adjusted; the listed base salary is $70,000–$85,000, with $75,000 variable compensation at target and on-target earnings of $145,000–$160,000. Variable pay is uncapped. The role is estimated to be 60–70% field-based and requires regular travel within the territory. Mileage reimbursement is provided at 76% of total business miles driven, processed monthly. Employment is contingent on a background check and motor vehicle record review after offer acceptance and before the start date.
