About The Position

The Nutrition Care Specialist provides comprehensive nutritional care to patients and residents with special needs or those identified as at nutritional risk. The primary responsibilities include patient assessment and reassessment, patient and family education, interdisciplinary communication, development of nutritional care plans, and documentation of all interventions in accordance with policy and accepted practice standards.

Requirements

  • Relevant education or training in nutrition, dietetics, or a related field.
  • Demonstrated ability to provide clinical care competently and effectively.
  • Strong communication skills to engage with patients, families, and healthcare team members.
  • Ability to document care and interventions in an organized and comprehensive manner.

Nice To Haves

  • Registered Dietitian Nutritionist (RDN) designation is preferred.

Responsibilities

  • Conduct thorough patient assessments to determine nutritional needs and risk factors.
  • Perform ongoing reassessments to monitor patients' nutritional status and make necessary adjustments to care plans.
  • Educate patients and their families about nutritional needs and dietary practices.
  • Collaborate effectively with interdisciplinary teams to ensure comprehensive patient care.
  • Develop and implement individualized nutritional care plans based on assessment findings.
  • Document all nutritional interventions and care plans accurately and in a timely manner.
  • Utilize appropriate technical and decision-making skills to provide high-quality clinical care.
