Nurse Practitioner - PRN (Post-acute)
About The Position
Eterna Primary Care is hiring a PRN Nurse Practitioner or Physician Assistant to provide post-acute care at centers in Mississippi. The clinician will serve patients in skilled nursing and assisted living facilities, helping bridge the transition from acute care to long-term wellness. The role focuses on high-quality, patient-centered care for complex and medically fragile populations, with the goals of reducing hospital readmissions and improving quality of life. Practitioners work autonomously while collaborating with physicians, nursing staff, therapists, consultant pharmacists, families, and facility leadership. The position includes schedule flexibility, dedicated documentation support, mentorship from physician leaders, and an on-call compensation plan.
Requirements
- Master’s or doctorate degree from an accredited Nurse Practitioner or Physician Assistant program.
- Current state license.
- Board certification.
- Active DEA registration.
- Independent yet collaborative approach, strong time-management skills, and compassionate communication that treats patients with respect.
Nice To Haves
- Background in geriatrics, post-acute care, or skilled nursing facility settings is strongly preferred.
Responsibilities
