Nurse Practitioner - PRN (Post-acute)
About The Position
Eterna Primary Care is seeking a nurse practitioner or physician assistant passionate about geriatric and post-acute care to provide care in skilled nursing and assisted living facilities. The practitioner will serve as a primary medical lead for patients transitioning from acute care, helping manage recovery and long-term wellness while reducing hospital readmissions and improving quality of life. The role involves comprehensive assessment and management of acute and chronic conditions in complex, medically fragile populations. The practitioner will use telehealth and evidence-based strategies, coordinate with hospitals, families, nursing staff, therapy teams, physicians, consultant pharmacists, and facility leadership, and support quality measures and cost-effective care aligned with patient goals. The position offers clinical autonomy, schedule flexibility, dedicated support staff, EMR documentation support, physician mentorship, and a collaborative, learning-oriented team environment.
Requirements
- Master’s or doctorate from an accredited nurse practitioner or physician assistant program.
- Current state license.
- Board certification.
- Active DEA.
- Independent yet collaborative approach, strong time-management skills, and compassionate communication that treats patients with profound respect.

- Experience in geriatrics, post-acute care, or skilled nursing facility (SNF) settings is strongly preferred.
