About The Position

Optum NY/NJ is seeking a Nurse Practitioner to join their team in Orange County, NY. As part of the Optum Care Delivery team, you will be an integral part of the vision to make healthcare better for everyone. The role involves providing care in the patient's home setting, with 5-8 patient visits per day including home visits and potentially video visits for established patients. It also includes regular interdisciplinary meetings to prioritize patient goals and collaboration with the patient's PCP to develop a care plan.

Requirements

  • Advanced Nurse Practitioner Certification in Primary Care, Geriatric, Adult, with unrestricted licensure in the state of NY at time of employment
  • Current and unrestricted DEA certificate required or ability to obtain
  • Valid driver’s license
  • Ownership of a reliable and insured vehicle, with the ability to travel within assigned territory to meet with patients
  • Comfortable working independently in a home-visit setting and clinic setting
  • Ability to communicate with patients, primary care team members and management staff
  • Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician
  • Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice
  • Advanced assessment and treatment plan implementation skills

Responsibilities

  • Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues, in collaboration with patient’s Primary Care Physician
  • Effectively identifies, evaluates, and addresses disease prevention and health promotion issues while administering quality patient care.
  • Maintains accurate medical records and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations.
  • Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.
