About The Position

WellBe Senior Medical is hiring a Nurse Practitioner to provide value-based, field-based primary care directly in patients’ homes in Scranton, Pennsylvania. The role focuses on autonomous, outcomes-based care for seniors, including whole-person care that considers social determinants of health. The practitioner will manage a dedicated patient panel with support from an interdisciplinary care team and will receive daily team check-ins, ongoing training, and one-on-one mentorship. The position offers a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, full benefits, and relocation availability. Employment is contingent on successful pre-employment drug screening and a background check. The company also conducts monthly FACIS checks on employees. WellBe does not provide employment-based visa sponsorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer visa sponsorship.
Successfully complete pre-employment drug screening and a background check; continued employment is contingent on applicable drug-free workplace requirements and satisfactory monthly FACIS checks.

Nice To Haves

Master’s degree focus in Adult, Geriatric, Acute Care, and/or Family practice.

Responsibilities

Conduct comprehensive primary care visits and physical examinations in patients’ homes.
Create longitudinal care plans to manage chronic disease in high-risk patients.
Partner with an interdisciplinary team to support seamless and safe care transitions.
Treat acute needs in the home to help prevent emergency room visits and hospitalizations.
