About The Position

WellBe Senior Medical is hiring a Nurse Practitioner (APP) to provide value-based primary care directly in patients’ homes in Scranton, Pennsylvania. The role emphasizes autonomous, outcomes-based care that addresses patients’ overall needs, including social determinants of health, and supports a dedicated patient panel with an interdisciplinary care team. The position offers a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, full benefits, and relocation assistance. The organization provides daily team check-ins, ongoing training, and one-to-one mentorship. Employment is contingent on successful completion of pre-employment drug screening and a background check. WellBe conducts monthly FACIS checks on employees, and ongoing employment may be subject to the company’s drug-free workplace testing policy. WellBe does not provide employment-based visa sponsorship. Employment is at-will unless otherwise specified by contract.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Pennsylvania.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without current or future employer sponsorship.
Successfully complete pre-employment drug screening and a background check.

Responsibilities

Conduct comprehensive primary care visits and physical examinations in patients’ homes.
Develop longitudinal care plans to manage chronic diseases in high-risk patients.
Collaborate with an interdisciplinary team to support seamless and safe care transitions.
Treat acute needs in the home to help prevent emergency room visits and keep seniors safe and healthy.
