About The Position

Matrix Medical Network is seeking a PRN Nurse Practitioner to provide in-home Comprehensive Health Assessments (CHAs) for adult and geriatric members. Visits can last up to one hour and involve reviewing and observing members’ current health, medical history, medication adherence, social environment, and other risks. The assessments provide insight into overall health and well-being, help improve quality of care, and may address multiple care gaps in a single visit. Providers deliver services at members’ residences, including private homes and Skilled Nursing Facilities. Assessment visits are scheduled according to provider availability and are conducted Sunday through Saturday, from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with a stated rate range of $90–$115 per assessment depending on state of residence. Providers who complete an average of 20–100 visits per month are described as earning $1,800–$11,500 per month. Matrix offers onboarding training, a preceptor, individual feedback, and access to continuing education. When business needs exist, the company may support providers in obtaining additional state licensure and credentialing. PRN providers may transition to full-time roles, which include a full-time sign-on bonus.
