•Onsite

About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home and facility-based Comprehensive Health Assessments for adult and geriatric members, including older adults and other at-risk individuals. Visits can last up to one hour and involve reviewing a member’s current health, medical history, medication adherence, social environment, and other risks. These assessments provide insight that may be difficult to capture in routine office visits and can help improve care quality and close multiple care gaps. Providers deliver services at members’ places of residence, including homes and Skilled Nursing Facilities. Scheduling is based on provider availability, with assessments conducted Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with the stated rate depending on the provider’s state of residence. Matrix provides onboarding, a preceptor, individual feedback, and access to continuing education. PRN providers may transition to full-time opportunities supporting a home location, state, or region.
