About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home Comprehensive Health Assessments for adults and geriatric members. Visits can last up to one hour and involve reviewing and observing a member’s health, medical history, medication adherence, social environment, and other risks. Assessments can help improve quality of care and identify opportunities to close multiple care gaps in a single visit. Providers deliver services at members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled around the provider’s availability, with assessments offered Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with rates ranging from $90 to $115 per assessment depending on state of residence. Providers who average 20 to 100 visits per month may earn $1,800 to $11,500 per month. Matrix provides onboarding training, a preceptor, individual feedback, and free access to continuing education. When business needs exist, the company may support providers in obtaining additional state licensure and credentialing. PRN providers may transition to full-time roles and receive a full-time sign-on bonus.
