About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home Comprehensive Health Assessments for adult and geriatric members. Visits can last up to one hour and may take place in members’ homes or Skilled Nursing Facilities. During each visit, providers review and observe the member’s health, medical history, medication adherence, social environment, and other risks. The assessments offer insight into overall health and well-being, help improve quality of care, and may close multiple care gaps in one visit. Providers conduct assessments and document findings according to Matrix guidelines and protocols. Visits are scheduled according to the provider’s availability, with assessments offered Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with the stated per-assessment rate depending on the provider’s state of residence. Matrix offers onboarding, preceptor support, feedback, and access to continuing education. PRN providers may transition to full-time roles when opportunities are available.
