Nurse Practitioner/Physician Assistant: Post-Acute & SNF (Part-Time)
About The Position
Theoria Medical is seeking a part-time Advanced Practice Provider to serve residents in local skilled nursing facilities. The role emphasizes continuity of care, building relationships with patients, and providing high-quality, patient-centered care to vulnerable populations. Providers work with autonomy and flexible scheduling, supported by clinical colleagues, technology, and resources. The position includes daily facility rounds, prescribing medications and treatment plans, addressing acute or emergent issues, and communicating with facility nursing staff. Providers are expected to attend scheduled shifts on site and follow clinical standards, facility policies, and regulatory requirements. Requirements
- Hold an active U.S. license.
- Nurse Practitioners must be board-certified by ANCC, AANPCB, PNCB, or AACN.
- Physician Assistants must be board-certified by NCCPA.
- Be qualified in a specialty such as Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine.
Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.
- Conduct daily rounding.
- Prescribe medications and treatment plans.
- Address acute and emergent issues.
- Communicate with facility nursing staff by phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
