Nurse Practitioner/Physician Assistant: Post-Acute & SNF (Part-Time)
About The Position
Theoria Medical is seeking a part-time Advanced Practice Provider to serve residents at local skilled nursing facilities. The role focuses on continuity of care, building relationships with patients over time, and providing high-quality, patient-centered care to vulnerable populations. Providers work with autonomy and flexible scheduling, supported by technology and a collaborative clinical team. The position uses a productivity-based compensation model, with estimated annual earnings of $42,000 to $118,000 and opportunities for annual performance and longevity bonuses. Providers receive a company-provided iPad with LTE for charting and can use an EHR, voice dictation and AI Scribe, and UpToDate. The role has no on-call or overnight requirements. The stated six-hour facility shifts apply to full-time positions.
Requirements
- Hold an active U.S. license.
- For Nurse Practitioners: hold board certification from ANCC, AANPCB, PNCB, or AACN.
- For Physician Assistants: hold board certification from NCCPA.
- Be able to perform the position's essential functions satisfactorily, with or without reasonable accommodation.

Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.
- Conduct daily rounding, prescribe medications and treatment plans, and address acute or emergent issues.
- Communicate with facility nursing staff by phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Comply with clinical standards, facility policies, and regulatory requirements.
