About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes to build trusted relationships and provide flexible, comprehensive, and personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen. The role primarily takes place in members’ private residences, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The role covers Springfield, Massachusetts, and Hampden County.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Perform additional diagnostic tests, such as spirometry or diabetic eye exams, when ordered by the member’s health plan.
Build trusted relationships with members and help connect them with care suited to their needs.
Provide Signify Health services, including Diagnostic & Preventive Services products.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Use an iPad and the clinical workflow for each evaluation.
