About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in Zanesville. The clinician visits health plan members in their homes to build trusted relationships, help improve health outcomes, and connect members with appropriate care. Visits last about 45 minutes and include time to answer health questions, review medication and medical history, and conduct a physical evaluation. Additional diagnostic tests may be provided when ordered by the member’s health plan, such as diabetic eye exams, spirometry, and bone density screening. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order laboratory tests, or change a member’s current treatment regimen. The role may also involve virtual visits and local travel, including travel within licensed states as needed. The primary workplace is members’ private residences. Home environments may include household pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform their duties safely and comfortably in these conditions. The position is full time, with anticipated hours of 40 per week. The typical pay range is $87,035.00–$187,460.00. The actual offer depends on factors including experience, education, geography, and other relevant factors. Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.
Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
