About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The position focuses on providing flexible, comprehensive, and personalized health evaluations directly to patients. The evaluations, lasting approximately 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the patient's health plan. Clinicians will use an iPad with an intuitive workflow and are not responsible for prescribing medication, ordering lab tests, or altering current treatment plans. The role requires flexibility to travel locally and within licensed states, and may include conducting virtual visits and providing various Signify Health services such as spirometry, diabetic eye exams, and bone density screenings.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.
Able to cover the primary location: Oneida County.
