About The Position

Matrix Medical Network is hiring a PRN provider to conduct in-home Comprehensive Health Assessments (CHAs) for adults, older adults, and other at-risk individuals. Visits may last up to one hour and involve reviewing and observing members’ health, medical histories, medication adherence, social environments, and other risks. Assessments can help improve quality of care and identify care gaps and chronic conditions that may otherwise go undiagnosed. Providers deliver services at members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled around provider availability, with assessments offered Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, with stated rates varying by state. The role includes onboarding, preceptor support, feedback, and access to continuing education. Matrix may support additional state licensure and credentialing when business needs exist. PRN providers may have the opportunity to transition to full-time roles.
