Matrix Medical Network•Town of Greece, NY
•Onsite

About The Position

Matrix Medical Network is seeking a PRN Nurse Practitioner or Physician Assistant to conduct comprehensive health assessments for adult and geriatric members in their homes or Skilled Nursing Facilities. Visits can last up to one hour and review members’ current health, medical history, medication adherence, social environment, and other health risks. These assessments can reveal needs that may be missed during routine office visits and may help close multiple care gaps in a single visit. Providers conduct assessments and document their work according to Matrix guidelines and protocols. They may also administer or order point-of-care and lab-based tests, make referrals, provide health education, and coordinate follow-up with members’ Primary Care Physicians and other members of the care team. Visits are scheduled according to provider availability, with assessments conducted Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments, at a stated rate of $90–$115 per assessment depending on state of residence. The posting says providers completing an average of 20–100 visits per month earn $1,800–$11,500 per month. PRN providers may transition to full-time opportunities; the posting notes that full-time roles receive a sign-on bonus.
