About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change members’ current treatment regimens.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. The role requires local travel and travel within licensed states as needed, as well as virtual visits when needed. The position covers Plattsburgh and the surrounding area and has an anticipated schedule of 29.5 hours per week.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services, such as spirometry and diabetic eye exams, when indicated by a member’s health plan.
• Use an iPad and clinical workflow for each evaluation.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated for the role.
