In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Richmond County, NY
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Richmond County, NY. The role focuses on building trusted relationships, helping members understand their health, and connecting them with appropriate care. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The clinician’s primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The role may also involve local travel and travel within licensed states, as needed, and virtual visits. The position averages 29.5 hours per week. Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with health plan members and help connect them to care suited to their needs.
- Use an iPad and the clinical workflow for evaluations.
- Travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
