About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit people in their homes to build trusted relationships and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role may include virtual visits and local travel within licensed states as needed. The position is primarily associated with Augusta, Maine, and is listed in Oxford, Maine.

• Board certification.

Responsibilities

• Conduct in-home health evaluations, typically lasting about 45 minutes.
