About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will provide flexible, comprehensive, and personalized health evaluations at patients' homes. The visit, lasting about 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The role involves travel locally and within licensed states, conducting virtual visits as needed, and providing diagnostic and preventive services.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to cover the following primary locations: Fort Collins MSA, Larimer County
