About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will provide flexible, comprehensive, and personalized health evaluations directly to individuals. The visits, lasting approximately 45 minutes, allow for one-on-one connection and addressing health questions. The evaluation includes a review of medications and medical history, a physical assessment, and potentially additional diagnostic tests if ordered by the health plan. Clinicians will utilize an iPad with an intuitive workflow and are not responsible for prescribing medication, ordering lab tests, or altering current treatment plans.

Board certification (required)
Able to cover the primary location: Portland, ME

Responsibilities

Conduct In-Home Health Evaluations.
Engage with individuals in their homes to bridge gaps in care.
Build trusted relationships to improve health outcomes.
Provide flexible, comprehensive, and personalized health evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Administer additional diagnostic tests as ordered (e.g., diabetic eye exams, spirometry).
Utilize an iPad and clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
