About The Position

Signify Health, part of CVS Health, is seeking a part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain pets, secondhand smoke, or other airborne allergens. Candidates must be able to perform duties safely in these environments.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).
Ability to cover the primary locations: Livingston, Rochester and surrounding area.

Responsibilities

Conduct In-Home Health Evaluations.
Engage with individuals in their homes to bridge gaps in care.
Build trusted relationships to improve health outcomes.
Provide flexible, comprehensive, and personalized health evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Administer diagnostic tests as ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
