About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct comprehensive health evaluations, including medication and medical history reviews, physical assessments, and potentially diagnostic tests. The goal is to provide flexible, comprehensive, and personalized health evaluations directly to individuals.

As an in-home clinician, the primary workplace is the private residence of health plan members. These environments can be diverse and may include household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be comfortable and safe performing duties in these varied conditions.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification.
• Able to cover the primary location: Marshall, MN.

Responsibilities

• Conduct In-Home Health Evaluations.
• Engage with individuals in their homes to bridge gaps in care.
• Build trusted relationships to improve health outcomes.
• Conduct medication and medical history reviews.
• Perform physical evaluations.
• Conduct additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
