Health Risk Assessments - Nurse Practitioner (NP) and Physician Assistants (PA)
About The Position
This flexible PRN opportunity is for Nurse Practitioners and Physician Assistants conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Clinicians work autonomously and provide assessments and preventive care education without ongoing treatment responsibilities. The position requires a minimum of eight full 7.5-hour days per month, with clinicians creating their own schedules. Travel is local, within the clinician’s county and nearby areas, roughly within 60 minutes of home. Documentation is completed using a company-provided iPad and a proprietary EMR system. Malpractice insurance and collaborating physicians are included. The role is offered as a 1099 contractor position, with streamlined onboarding that may allow clinicians to begin in three to four weeks.
Requirements
- Active Nurse Practitioner or Physician Assistant license.
- Pediatrics, PMHNP, and Women’s Health specialties are excluded.
- Availability for at least eight full 7.5-hour days per month.
- Willingness to travel within roughly 60 minutes of home.
Responsibilities
- Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
- Review each patient’s medical history, current medications, lifestyle, and key social factors.
- Conduct basic physical and mental health evaluations.
- Provide preventive care education without ongoing management.
- Travel locally to patients in the clinician’s county and nearby areas.
