About The Position

This flexible PRN opportunity is for Nurse Practitioners and Physician Assistants conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Clinicians work autonomously and provide assessments and preventive care education without taking on ongoing treatment responsibilities. The role has an eight-day monthly minimum and allows clinicians to create their own schedules. There are no prescribing duties, on-call shifts, or follow-ups. Documentation is completed using a company-provided iPad and the company’s proprietary EMR system. Malpractice insurance and collaborating physicians are included. The role is offered as a 1099 contractor position and requires local travel to patients in the clinician’s county and nearby areas, generally within roughly 60 minutes of home. Onboarding may take three to four weeks.

Requirements

• Active Nurse Practitioner or Physician Assistant license.
• Pediatrics, Psychiatric-Mental Health Nurse Practitioner (PMHNP), and Women’s Health specialties are excluded.
• Availability for at least eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.

Responsibilities

• Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
• Review patients’ medical histories, current medications, lifestyles, and key social factors.
• Conduct basic physical and mental health evaluations.
• Provide preventive care education; ongoing management is not required.
• Travel locally to patients in the clinician’s county and nearby areas.
