Health Risk Assessments - Nurse Practitioner (NP) and Physician Assistants (PA)
About The Position
Flexible PRN opportunity for Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. The role is autonomous and focuses on assessment and preventive care education, without ongoing treatment responsibilities. Clinicians create their own schedule, with a minimum availability of eight full 7.5-hour days per month. The position includes competitive pay with performance incentives, malpractice insurance, and collaborating physicians. Documentation is completed through a company-provided iPad and the company’s proprietary EMR system. The role is a 1099 contractor position and involves local travel to patients in the clinician’s county and nearby areas, roughly within 60 minutes of home. Onboarding may take 3 to 4 weeks.
Requirements
- Active Nurse Practitioner or Physician Assistant license.
- Pediatrics, PMHNP, and Women’s Health specialties are excluded.
- Availability for at least eight full 7.5-hour days per month.
- Willingness to travel roughly 60 minutes from home.
Responsibilities
- Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
- Review each patient’s medical history, current medications, lifestyle, and key social factors.
- Conduct basic physical and mental health evaluations.
- Share preventive care education; ongoing management is not required.
- Travel locally to patients in the clinician’s county and nearby areas.
