About The Position

This flexible PRN opportunity is for Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. The role offers schedule flexibility and independent work, with no ongoing treatment responsibilities, prescribing, on-call shifts, or follow-ups. Clinicians must be available for at least eight full 7.5-hour days per month and travel to patients within their county and nearby areas, roughly 60 minutes from home. The position is offered as a 1099 contractor role. Onboarding may be completed in three to four weeks.

Requirements

• Active Nurse Practitioner or Physician Assistant license.
• NP licenses in Pediatrics, Psychiatric-Mental Health, and Women’s Health specialties are excluded.
• Availability for at least eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.

Responsibilities

• Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
• Review patients’ medical histories, current medications, lifestyles, and relevant social factors.
• Conduct basic physical and mental health evaluations.
• Provide preventive care education without ongoing management.
• Travel locally to patients in the clinician’s county and nearby areas.
