Health Risk Assessments - Nurse Practitioner (NP) and Physician Assistants (PA)
About The Position
This flexible PRN opportunity is for licensed Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. The role offers schedule flexibility and autonomous work, with no ongoing treatment responsibilities. Clinicians must be available for at least eight full 7.5-hour days each month and travel to patients within their county and nearby areas, roughly within 60 minutes of home. The role is offered as a 1099 contractor position. Onboarding may take 3 to 4 weeks. The position includes competitive pay and performance incentives, malpractice insurance, and collaborating physicians. Documentation is completed using a company-provided iPad and the company’s proprietary EMR system. There are no prescribing duties, on-call shifts, or follow-ups.
Requirements
- Active Nurse Practitioner or Physician Assistant license.
- License must not be in Pediatrics, Psychiatric-Mental Health Nurse Practitioner (PMHNP), or Women’s Health specialties.
- Availability for at least eight full 7.5-hour days per month.
- Willingness to travel roughly within 60 minutes of home.
Responsibilities
- Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
- Review each patient’s medical history, current medications, lifestyle, and relevant social factors.
- Conduct basic physical and mental health evaluations.
- Provide preventive care education without ongoing patient management.
- Travel locally to patients within the assigned county and nearby areas.
