About The Position

NIVA Health is seeking a full-time, field-based Licensed Practical/Vocational Nurse to provide advanced wound care in patients’ homes. The nurse will travel locally, deliver patient-centered wound care, collaborate with an interdisciplinary care team, educate patients and families, and document care in the EMR. The role includes ongoing training and opportunities for professional growth. Estimated hourly earnings are $35–$39, based on completed visit volume and hours worked. Pay is W-2 hourly for all hours worked, including patient care, charting, and travel time between patients; nurses also receive $25 per point, overtime in accordance with applicable law, and mileage reimbursement for business travel between patient locations. The schedule is typically weekday coverage, with 25–50 visits and 40–45 hours per week depending on route and volume.

Requirements

• Active LPN/LVN license in the state of practice.
• Passion for wound care and patient-focused healthcare.
• Strong communication and organizational skills.
• Ability to work independently in a mobile setting.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide advanced wound care therapies, including low-frequency, non-contact ultrasound (MIST).
• Travel locally to patients’ homes while following safety and infection-control standards.
• Collaborate with an interdisciplinary care team to support excellent patient outcomes.
• Educate patients and families about wound care and recovery.
• Complete accurate, timely documentation in the EMR.
• Participate in ongoing training and pursue advancement opportunities.
