About The Position

The Memory Care Support Partner works with residents in a memory care neighborhood, creating human connections and supporting residents through their daily routines. The role involves learning residents’ interests and abilities, collaborating on engagement plans, and patiently implementing strategies. The Support Partner helps prepare resident spaces and shared areas for daily activities, meals, events, and nighttime routines, and partners with residents throughout the day. Responsibilities also include documenting resident care and changes in condition and assisting with orientation of team members assigned to memory care neighborhoods. This is a full-time position at Carriage Court in Hilliard, Ohio, scheduled Monday through Friday from 7:00 a.m. to 3:00 p.m. Pay is $18.50–$21.50 per hour.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive, energetic attitude and enjoy working with residents.
Have computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be able to assist residents with sitting, standing, and walking, and assist people after a fall.
Be criminally cleared.

Responsibilities

Create human connections with residents and provide empathetic support during vulnerable moments.
Discover residents’ interests, abilities, and individual qualities.
Collaborate to create appropriate engagement plans and patiently implement strategies.
Prepare dining rooms, libraries, resident rooms, and other spaces for meals, events, and daily routines.
Partner with residents to support all facets of daily life, from waking to bedtime.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
