About The Position

The Memory Care Support Partner works with residents in a memory care neighborhood, creating human connections through empathy and supporting residents throughout their daily routines. The role involves learning about residents’ interests and abilities, collaborating on engagement plans, and using patient strategies to help residents participate successfully. The partner prepares and arranges spaces for residents’ activities and routines, from dining and events to nighttime preparation, and supports residents from waking through bedtime. Responsibilities also include documenting resident care and changes in condition and assisting with orientation for team members assigned to memory care neighborhoods. This is a full-time position at Carriage Court in Hilliard, Ohio. The scheduled shift is 11:00 p.m. to 7:00 a.m. on Monday, Tuesday, Wednesday, Thursday, and Sunday. The listed pay range is $18.50–$21.50 per hour.

Requirements

Must be at least 18 years of age.
High school diploma or equivalent.
Ability to read, write, understand, and communicate in English with residents.
Positive and energetic attitude and enthusiasm for working with residents.
Computer skills sufficient to print documents and enter resident information.
Ability to stand, walk, bend, kneel, and stoop throughout the day.
Ability to frequently lift and/or move items weighing up to 50 pounds.
Must be criminally cleared.

Responsibilities

Create empathetic human connections with residents, particularly during vulnerable moments.
Observe residents’ interests and abilities and collaborate on appropriate engagement plans and strategies.
Prepare resident and community spaces for meals, events, and daily or nighttime routines.
Partner with residents in daily life from waking through bedtime.
Document resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
Assist residents with sitting, standing, and walking, including assisting people after a fall.
